Please fill out the forms below in as much detail as you can, and bring them to your first visit. Plan to arrive 30 minutes before your first appointment to finish paperwork and settle in.

Your first visit
Your first visit with Dr. Julie or Dr. Bryan starts with an evaluation of your symptoms and, in your own words, your health needs. You'll talk through your challenges, concerns and goals. Dr. Julie draws on chiropractic, Traditional Chinese Medicine (acupuncture) and lab analysis to address your needs.
Insurance & payment
